EACCME® Participant’s Evaluation Form

Thank you for attending the 23rd World Congress on Dental Traumatology (WCDT 2026), held on September 3-5, 2026 in Warsaw, Poland.

The CME/CPD certificate including EACCME® credits will be available after completing the online evaluation and credit-claiming procedure. The process takes about 5 minutes. We thank you for your feedback, as it is an important part of CME/CPD accreditation and helps improve future educational offerings.

Before 23 September 2026:

    1. Complete the online evaluation form.
    2. Enter your first and family name and the email address used for congress registration during the credit-claiming process.
    3. Complete and submit the credit claim form. If you do not want to complete the form in one sitting, you can save your progress and continue later. To do so, scroll down, click “Save”, and enter your email address. You will receive a link that will allow you to resume filling out the form later.
    4. Once you have completed the evaluation form, your certificate will be sent to you by email within the following days.
    5. Please note that if you did not attend the conference, completing this form will not result in a valid certificate being issued.
